Practice Policies

New clients are required to complete an intake packet via the Headway and/or Simple Practice platform in advance of their first session. This packet includes a review and acknowledgment of this practice's policies. A sample of these policies are accessible here:

Controlled Substances Policy and Agreement

If a controlled substance (e.g., Suboxone, stimulants, benzodiazepines) is prescribed, a separate, medication specific agreement is required. This agreement will be reviewed during your appointment before the prescription is issued.

The agreement outlines specific requirements, which may include:

  • Initial and Random Drug Screening: Patients may be required to undergo urine or blood drug screening before the initial prescription and randomly throughout treatment. Random screenings must be completed within 24–48 hours of notice.

  • Single Prescribing Provider: Patients must agree not to seek or have active prescriptions for similar controlled medications from outside providers, particularly those that could cause adverse reactions (e.g., respiratory depression, increased risk of death) when combined with the prescribed medication.

  • Consequences of Illicit Substance Use: Drug screening tests that indicate the use of illicit/illegal substances (e.g., heroin, cocaine, PCP) may result in consequences such as dismissal from the practice, reduction/tapering of the controlled substance, or discontinuation of the prescription.
